Required Documents
To begin clinical placement, students must submit complete and valid documentation before any site requests can be processed. In practice, missing or outdated paperwork is the most common cause of delays, so accuracy and timeliness are essential.
Documents Needed
- Resume or CV – Required for nurse practitioner (and some advanced) programs to review experience and academic background
- RN license verification – Must be active and unencumbered, verified through the state board
- Malpractice insurance – Proof of individual professional liability coverage for the full rotation period
- Background check (if required) – Some clinical sites require this prior to onboarding
- University placement forms – Official school documents required for clinical approval
- Immunization records (if applicable) – TB test, vaccines, or titers, depending on site requirements
Key Point to Understand
Clinical sites will not approve placement until all documents are verified and compliant. Even one missing or expired item can pause the process. Once everything is approved, we move forward with submitting placement requests and matching you with an appropriate clinical site and preceptor.
